Verapamil in Diabetes

چکیده
Figure 1: Models of thioredoxin‐interacting protein action: (a) Role of thioredoxin‐interacting protein in the thioredoxin system. Thioredoxin‐interacting protein binds and inhibits the reduced form of thioredoxin, thereby functioning as a rheostat that modulates both redox status and reactive oxygen species‐mediated signaling to regulate metabolism and other cellular processes. (b) Proposed role of thioredoxin‐interacting protein in type 2 diabetes. Chronic glucose load on the pancreas, triggering thioredoxin‐interacting protein‐mediated β cell failure and overt diabetes. Adapted with permission from[1] b a Sir, Loss of pancreatic β cells is a pathological hallmark of both type 1 and type 2 diabetes mellitus; however, no specific therapy targeting this defect is yet available. A paradigm shift with such a molecule has always been awaited. Verapamil – a nondihydropyridine calcium channel blocker used in the treatment of hypertension, angina, and tachyarrhythmias, particularly atrial fibrillation – has been observed to show some hope in preventing β cell loss in diabetics by inhibiting thioredoxin-interacting protein (TXNIP).

Purpose: The aim of this study was to evaluate the effect of verapamil and nifidipine on diabetic kidney in rats. Materials and Methods: In this experimental study, 6 groups of rats were studied, group I; intact (sham) group II; diabetic (control), group III and IV; diabetic treated with verapamil or nifedepine group V and VI normal rat which recieved verapamil or nifedipine. Diabetic rats (co...
متن کاملManaging hypertension in diabetic patients – focus on trandolapril/verapamil combination
Hypertensive diabetes individuals are at higher risk for cardiovascular events and progression to end stage renal disease. Several well conducted clinical trials indicate that aggressive treatment of hypertension in individual with diabetes reduces these complications. Combinations of two or more antihypertensive drugs are frequently required to reach the target blood pressure and to improve th...
متن کاملOpposite effect of diabetes mellitus induced by streptozotocin on oral and intravenous pharmacokinetics of verapamil in rats.
The aim of this study was to report the effect of diabetes mellitus on the pharmacokinetics of verapamil in a route-dependent manner. Diabetes in rats was induced by streptozotocin. Plasma concentrations of verapamil and its metabolite, norverapamil, were measured after oral (10 mg/kg) or intravenous (1 mg/kg) administration. The concentrations of verapamil in portal plasma after oral administr...
متن کاملPreventing microalbuminuria in type 2 diabetes.
BACKGROUND The multicenter double-blind, randomized Bergamo Nephrologic Diabetes Complications Trial (BENEDICT) was designed to assess whether angiotensin-converting-enzyme inhibitors and non-dihydropyridine calcium-channel blockers, alone or in combination, prevent microalbuminuria in subjects with hypertension, type 2 diabetes mellitus, and normal urinary albumin excretion. METHODS We studi...
متن کاملClinical outcomes in the diabetes cohort of the INternational VErapamil SR-Trandolapril study.
The INternational VErapamil SR-Trandolapril study (INVEST) had 6400 of 22,576 (28.3%) participants with diabetes at entry. The objectives of this prespecified analysis were to compare antihypertensive treatment strategies in the diabetes cohort (verapamil SR-based [n=3169] versus atenolol-based [n=3231]) and identify predictors for the primary outcome (a composite of first occurrence of all-cau...
متن کاملCardiotoxicity of verapamil in renal failure: a case report and review of the literature
We present a case of a 76-year-old diabetic patient on verapamil with undiagnosed renal failure presenting with collapse and severe life threatening bradyarrhythmias. She responded well to inotropic support and calcium supplementation.
